Emergency Cards (Can be obtained at the nurse's office)

Each year parents are required to complete an emergency care information card to indicate an "immediate" emergency referral system for their child.  In the event a parent cannot be contacted, two persons should be designated on the card as contact persons.  The contact persons should reside in the immediate area and are willing to assume temporary care and supply transportation for your child.  Only persons indicated on the emergency care information card will be contacted.
